However, his life took a tragic turn under the care of a caregiver approved by social workers. In 2021, Colby suffered a fatal nine-minute beating, prompting a scathing report released by British Columbia’s representative for children and youth.

Minister Grace Lore of children and family services expressed deep sorrow over Colby’s ordeal, acknowledging the failure to protect him and his family. She pledged comprehensive reforms across ministries to ensure the safety of vulnerable children, emphasizing collaboration among health, education, social services, and justice sectors.

The report highlighted systemic failures, including missed warnings from school staff and doctors about Colby’s well-being, which were not acted upon by child welfare services.

Lore affirmed her commitment to breaking down barriers that prevent professionals from raising concerns about children at risk, underscoring the urgent need for a responsive and interconnected support system.

Indigenous leaders welcomed Lore’s pledge to address systemic issues outlined in the report titled “Don’t Look Away,” urging swift action on its recommendations to prevent future tragedies. They emphasized that previous reports have yielded numerous recommendations that have yet to be implemented, highlighting the ongoing need for reform in child welfare policies.
